1. Question: What exactly is this questionnaire ?
1. Answer: The questionnaire contains more than 140 questions which meet the scientific criteria to provide an early pre-diagnosis of your sleep quality. In the evaluation, you will receive information about a whole series of subjects: Besides an introduction to some general clarifications, you will also find the following categories explained in detail: ApneaAccompanying illnesses, risk factors for sleep apnea Insomnia Parasomnias Narkolepsy Restless Legs Syndrome Sleep hygiene Sleep quality Vigilance Epilepsy Depression to the top 2. Question: Why must I register ?
2.Answer: A registration is necessary in order to avoid abuse of email addresses and to be able to send you the evaluation of the questionnaire. to the top 3. Question: How do I fill out the questionnaire ?
3. Answer: You can fill out the four part questionnaire online after your registration with sleep-test.de and save it at any time. The filling in process can be interrupted at any time and when you want to start up again, you can login once more. An evaluation takes place, of course, only if all questions of all four parts were answered by you completely. In case you don’t have a flat rate and would like to work offline, or simply prefer to read the questions on paper, you can also download the complete questionnaire as a PDF file (see service). After filling out the questionnaire on paper, please then transfer (transcribe) the answers onto the online form. It is currently only possible to submit paperless questionnaires for evaluation. to the top 4. Question: What does the evaluation cost ?
4. Answer: The costs for the evaluation of the questionnaire amount to 9.90€. You can read the evaluation on the screen and print it out. Additionally, you can have the evaluation sent to you by email. to the top 5. Frage: How do I get the results ?
5. Answer: The evaluation of the questionnaire occurs immediately if all questions were answered. You can print out the results immediately (a special print version is available with an output in standard A4 format). Simply use the print function in your browser. This assumes a the use of a modern browser (CSS 2.0 required). You can also request the evaluation be sent to you by email. Just scroll completely to the end of the screen output and press the intended button. to the top 6. Frage: What method are available to pay ?
6. Answer: Payment is possible with the two popular and secure systems ClickandBuy and PayPal. Both offer secure online payments. You must go through the absolutely free of charge registration process with ClickandBuy or PayPal before filling out the questionnaire. 7. Answer: You can take the evaluation of the questionnaire to your family doctor and discuss any further necessary steps with him.
